Terms and Conditions When you purchase a set of four Yokohama tires you may be eligible to receive a $75 Subaru Yokohama Prepaid Mastercard® by mail. Offer valid on qualifying tires purchased from Subaru retailers and picked-up or installed between 12:00 a.m. EDT October 1, 2025 and 11:59 p.m. EST November 30, 2025. Please confirm eligibility with your Subaru retailer. To receive the Subaru Yokohama Prepaid Mastercard, you must submit: • (i) an official rebate form • (ii) an original or legible copy of the invoice for the qualifying Yokohama tires to the following address: Subaru Yokohama Rewards Attn: Rewards Center STR_Y537 7015 Vorden Parkway South Bend, Indiana 46628 The name on the rebate form or online submission and invoice must match. Mail-in submissions must be postmarked by December 10, 2025. Mail-in or online redemptions must be completed and received by the Rewards Center by December 14, 2025. Please allow for up to eight weeks after the submission deadline for delivery of the prepaid Mastercard. To check the status of the prepaid Mastercard, please contact the Rewards Center by phone at 888-741-0223 ext 4400. Prepaid card will be issued in the name on the redemption form only. Please retain copies of all materials you submit. All submissions become the property of the Promotion Administrator and will not be returned. The Promotion Administrator is not responsible for any lost, late, damaged, misdirected, postage-due, incomplete, incorrect, or illegible mail. You must be a legal U.S. resident, 18 years of age or older to be eligible for this offer. Offer valid only in the 50 United States and the District of Columbia (includes APO/FPO addresses).
